Placement Description
At River Valley Counseling Center I interned in the school-based program and served as a school-based clinician. As a school-based clinician I provided weekly individualized psychotherapy to children ages 11-18 diagnosed with DSM 5 diagnosis. These diagnosis included Post-Traumatic Stress Disorder, Major Depressive Disorder, Oppositional Defiant Disorder, Attention Deficit Hyperactivity Disorder, Adjustment Disorder with Depressed Mood, And Adjustment Disorder with Anxious Mood. To foster best practice techniques I created strength based treatment plans and constantly facilitated communication between clients, family members, and school personnel. After the completion of my internship I was hired as a school-based clinician where I carried a caseload of 12 to 15 client's and completed intakes. Within these intake sessions I completed multiple assessments and addendums, such as comprehensive assessments, risk assessments, mental health statues assessments, and assessed needs assessments.
Weekly Progress Notes
Included in my duties at River Valley was to complete weekly progress notes where I reported the client's progress towards goals, goals addressed within the treatment plan, therapeutic interventions delivered in session, client's response to intervention, and plan for next session. Below I have attached examples of my progress notes throughout the year.

Assessments
At River Valley many different assessments were completed to evaluate the state of the client receiving treatment. These assessments include but are not limited to The Child and Adolescent Needs and Strengths (CANS) Assessments, Comprehensive Assessments, and Risk Assessments.
